Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol.30,No.52, December 23, 2004 issue of the Florida Administrative Weekly.

The change is in response to continued written comments submitted by the staff of the Joint Administrative Procedures Committee (JAPC). The Board published a two (2) Notices of Change; the first in Volume 31, No. 22, of the June 3, 2005, Florida Administrative Weekly; and the second in Volume 31, No. 35, of the September 2, 2005, Florida Administrative Weekly. The following additional change to address the JAPC concerns is as follows:

Subsection (4)(a) shall be changed to read “(a) Application for renewal as a licensed anesthesiologist assistant must be made upon a form supplied by the Department, and incorporated in Rule 64B8-1.007, F.A.C.”
