59G-13.031: Aged and Disabled Adult Waiver Services Procedure Codes and Fee Schedule
The Agency For Health Care Administration announces a hearing to which all persons are invited.

GENERAL SUBJECT MATTER TO BE CONSIDERED: Transition Case Management. This hearing is being held to discuss adding Transition Case Management to the Aged and Disabled Adult Waiver Services Coverage and Limitations Handbook, May 2009 and adding Nursing Home Transition Case Management to the Aged and Disabled Adult Waiver Procedure Codes and Fee Schedule, May 2009. Transition Case Management services can be provided to Medicaid eligible individuals who reside in a nursing facility and wish to transition into a less restrictive environment within the community. This service can be used to assess, evaluate, plan, and coordinate the services needed by a potential nursing home transition candidate. The provider qualifications and the reimbursement rate for Transition Case Management will remain the same as currently provided by case management services under the waiver.
