65C-35.001: Definitions

Notice is hereby given that the following correction has been made to the proposed rule in Vol. 36 No. 3, January 22, 2010 issue of the Florida Administrative Weekly.

Subsection 65C-35.001(15), F.A.C., makes reference to the “Medical Report” form CF-FSP5339 dated October 2009, which is incorporated by reference. The form date is incorrect and should read January 2010.
