PURPOSE AND EFFECT: The Board proposes the review the application form and makes any necessary changes.

Any person who wishes to provide information regarding a statement of estimated regulatory costs, or provide a proposal for a lower cost regulatory alternative must do so in writing within 21 days of this notice.

(1) A properly completed application shall be submitted on Department of Health Form DH-MQA 1116, 12/09 03/09, adopted and incorporated herein by reference as this Board’s application and available on the web at www.doh.state.fl.us/mqa. To complete the application attach the appropriate fees and supporting documents and submit it to the Board Office.
