PURPOSE AND EFFECT: The purpose of the proposed amendment is to incorporate by reference the revised Florida Medicaid Hearing Services Coverage and Limitations Handbook, January 2009. The coverage and limitations handbook revisions include policy clarifications, place of service clarification, additional definitions, and new fiscal agent information. The effect will be to incorporate by reference in the rule the Florida Medicaid Hearing Services Coverage and Limitations Handbook, January 2009.

(2) All physicians, audiologists and hearing aid specialists enrolled in the Medicaid program must be in compliance with the provisions of the Florida Medicaid Hearing Services Coverage and Limitations Handbook, January 2009 July 2006, which is incorporated by reference, and the Florida Medicaid Provider Reimbursement Handbook, CMS-1500, which is incorporated by reference in Rule 59G-4.001, F.A.C. Both handbooks are available from the Medicaid fiscal agent’s Web Portal website at http://mymedicaid-florida.com floridamedic aid.acs-inc.com. Click on Public Information for Providers, then on Provider Support, and then on Provider Handbooks.
