RULE NO: RULE TITLE
64D-3.046: Immunization Requirements: Public and Nonpublic Schools, Grades Preschool, and Kindergarten Through 12, and Adult Education Classes
PURPOSE AND EFFECT: The Bureau of Immunization proposes an amendment to update forms and guidelines.
SUBJECT AREA TO BE ADDRESSED: The subject areas to be addressed include an updated DH 680 Form (Certification of Immunization) and the Immunization Guidelines for Florida Schools, Childcare Facilities and Family Day Care Homes. The guidelines will be updated to require a tetanus-diphtheria-acellular-pertussis (Tdap) vaccination rather that a tetanus-diphtheria (Td) vaccination at 7th grade entry to be implemented with the 2009-2010 school year.
