64B10-11.001 Application for Licensure Fee.
Any person desiring to be licensed as a nursing home administrator shall apply to the Board of Nursing Home Administrators. The application shall be made on the Application For Nursing Home Administrators Examination Endorsement/Temporary form DH-MQA-NHA002 (revised 10/07 9/04), hereby adopted and incorporated by reference, and can be obtained from the Board of Nursing Home Administrators’ website at http://www.doh.state.fl.us/mqa/nurshome/index.html or the Division of Medical Quality Assurance Call Center by calling (850)488-0595.
