60BB-3.029: Public Use Forms
PURPOSE AND EFFECT: This rule sets forth two forms to be used by the Agency for Workforce Innovation in the administration of its Unemployment Compensation Program. AWI Form AWA-01 (Rev. 3/2005) is used to process request for confidential unemployment compensation records and AWI form ERWC (New 02/06) implements the statutory provisions regarding release of confidential unemployment compensation information in workers’ compensation cases set forth in Sections 443.171(5), 443.1715(1), and 443.1715(2)(b)1., Florida Statutes.
SUMMARY: Rule 60BB-3.029 sets forth forms to be used by the Agency for Workforce Innovation to aid in the administration of its Unemployment Compensation Program. AWI Form AWA-01 (Rev. 3/2005) is used to process request for confidential unemployment compensation records and AWI form ERWC (New 02/06) implements the statutory provisions regarding release of confidential unemployment compensation information in workers’ compensation cases set forth in Sections 443.171(5), 443.1715(1), and 443.1715(2)(b)1., Florida Statutes.
SUMMARY OF ESTIMATED REGULATORY COSTS: No Statement of Estimated Regulatory Cost was prepared.
Any person who wishes to provide information regarding a statement of estimated regulatory costs, or provide a proposal for a lower cost regulatory alternative must do so in writing within 21 days of this notice.
