NOTICE IS HEREBY GIVEN that on June 20, 2011, the Board of Optometry, received a petition for waiver or variance filed by Courtney L. Case, O.D., from Rule 64B13-4.001, F.A.C., with regard to the licensure requirement of Rule 64B13-4.001, F.A.C., that applicants submit scores on the national licensing exam taken no longer than 5 years prior to application.
