NOTICE OF THE POSTING OF INTENDED AWARDS

FOR RURAL EMERGENCY MEDICAL SERVICES

MATCHING GRANTS (90% and 75% GRANTS)

GRANT TITLE: Florida Emergency Medical Services Rural Matching Grant Program

The list of intended grant recipients and those not intended for awards is posted outside of: Conference Room 315P, Third Floor, State Office Building, 4025 Esplanade Way, Tallahassee, Florida 32311-7829, beginning on July 1, 2011, 8:00 a.m. and also will be posted on the Internet at: http://www.fl-ems.com/Grants/Grants.html beginning on July 1, 2011.

The list also will be provided by mail, e-mail, or facsimile (Fax) by making a request directed to: State EMS Matching Grant Administrator, Bureau of Emergency Medical Services, Department of Health, 4052 Bald Cypress Way, Bin #C18, Tallahassee, Florida 32399-1738, (850)245-4440, extension 2734, Fax: (850)245-4378 or e-mail: Alan_VanLewen@doh.state.fl.us. Any entity whose substantial interests have been affected by this decision may file a petition for an administrative hearing as provided in Sections 120.569 and 120.57, F.S.

The petition must be in writing and conform to the requirements of Rule 28-106.201 or 28-106.301, F.A.C., as applicable.

Mediation pursuant to Section 120.573. F.S., is not available for this action. A written petition must be filed within twenty-one (21) days of the date of award posting. The term “filed” means received by the: Agency Clerk, Department of Health, 4052 Bald Cypress Way, Bin #A02, Tallahassee, FL 32399-1703. Written petitions may be hand-delivered to: Agency Clerk, 2585 Merchants Row Blvd., Prather Building, Suite 110, Tallahassee, FL.
