Form and Marketing Standards

PURPOSE:

To adopt the standard disclosure form required to be used in the Cover Florida Health Care Access Program established pursuant to Section 408.9091, Florida Statutes.

SUMMARY:

This rule adopts the standard disclosure form required by the Cover Florida Health Care Access Program established pursuant to Section 408.9091, Florida Statutes. Form OIR-B2-2004 (New 3/2010) is required to be provided to consumers purchasing Cover Florida Plan Coverage. The form provides important disclosures concerning terms of renewal, termination of coverage, portability, grace period, reinstatement, premium changes, preexisting conditions, cost sharing requirements and provides a schedule of services that are not covered and specific plan exclusions.
