RULE:

61G18-12.019

Reinstatement of a Null and Void Licensee Fee

PURPOSE:

The purpose and effect of the rule is to establish the fee amount and application forms for reinstatement of a null and void license.

SUMMARY:

The rule established a fee of $260.00 and incorporates forms DBPR PRO 4951, DBPR PRO 4952, AND DBPR PRO 4953 which are to be utilized in the reinstatement of a null and void license.
