Out-of-State Telehealth Provider Registration

PURPOSE:

To eliminate the obsolete telehealth survey requirement and provide a form for out-of-state telehealth providers to register with the appropriate board, or the department, if there is no board, prior to providing telehealth services to patients located in this state, as required by recently enacted legislation.
