Application for Licensure and Licensure Requirements for Anesthesiologist Assistants

PURPOSE:

The proposed rule amendment incorporates the revised application form into the rule which deletes the question regarding the applicant’s place of birth; deletes the civil judgment question and adds the question pertaining to the statement from FDLE and privacy statement from the FBI.

SUMMARY:

The proposed rule amendment incorporates the revised application form into the rule.
