Nonresident Pharmacy Permit - Mandatory Notification for Change in Location, Change in Pharmacy Name, Change in Corporate Officer and Change in PDM

PURPOSE:

The Board proposes the rule promulgation to create a rule regarding instructions for a change in locations, change in pharmacy name, and change in corporate officer and PDM for a nonresident pharmacy permit holder.
