PURPOSE:

The Board proposed the rule amendment to update and streamline the application form.

SUMMARY:

Changes to the application remove the question asking for birth place, notify applicants that email addresses are optional, and inform applicants of procedures to follow if they fail the exam 3 times within 12 months.
