Application, Certification, Registration, and Licensure Fees

PURPOSE:

The proposed rule amendment is intended to set forth a fee for those who are licensed pursuant to Section 458.3137, Florida Statutes.

SUMMARY:

The proposed rule amendment sets forth a fee in the amount of  $200 for those physicians who are licensed pursuant to the provisions of Section 458.3137, F.S.
