Notification of Insured's Rights and Standard Disclosure Form; Personal Injury Protection Benefits

PURPOSE:

To update and revise Form OIR-B1-1149 “ Notification of Personal Injury Protection Benefits” in accordance with revisions to the PIP law as amended by HB119 (Chapter 2012-197, Laws of Florida).

SUBJECT:

Amendments for Form OIR-B1-1149 which is required to be provided to insureds as set forth in section 627.7401, Florida Statutes.
