Forms Adopted

PURPOSE:

To update and edit the contents of the forms and instructions used by Life and Health insurers to make electronic form filings via the Office’s I-File system.

SUMMARY:

Form OIR-B2-1507 “Universal Standardized Data Letter” and OIR-B2-1505A “Instruction Sheet” are being revised to provide for more efficient generation of data processing for review by the Office of form and rate filings submitted by insurers. The definitions used in the instructions are being updated and made consistent.
